If an individual has several different healthcare providers, they may simply give us their names, addresses and telephone numbers and we will do our best to request their records on their behalf. Once received we will add them to their personal health record. This service is free for active subscribers to our service. To use this free service click the link “Get My Records” in the Account Manager to get started. Patients will be required to download a medical records release form, complete and sign it and either upload it or fax it to us prior to requesting records.
