Patient Registration

New registrants must first complete a short registration form on this Web site. This form will ask basic information such as name, address, telephone number and email address. Registrants are required to create a secure password and answer a series of security questions. Upon submission of the registration form, the registrant will receive a confirmation email as well as any additional instructions on completing their registration.

Creating Patient Personal Health Record (PHR) with HealthRecordExpress™

A personal health record, commonly known as a PHR, is a complete patient health profile that includes a patient self-assessment, physicians records, lab reports and prescription records. Newly registered patients are guided step-by-step to create their online personal health record. Patients may manage their medical history and records from their computer via their Web Browser.

Selecting a Physician

Southwest Medical Group's network physicians are located throughout the country and specialize in many areas of medicine. Once a new patient completes their personal health record a patient care specialist will help them choose a physician located in their area.

Requesting a Consultation with the Physician

Once a physician's office has been selected the patient may request a consultation with the physician. The patient will complete a consultation request that will ask important questions about their medical condition. The physician will review the patient request and inform the patient whether an in-person visit, telephonic consultation or secure email consultation is required. Southwest Medical Group will then schedule the patient for the type of consultation the physician has selected.

All network physicians require patients to be seen in-office at the physician's discretion for physical exams and medical evaluations before telephonic or email consultations can be provided. Physician consultations scheduled through Southwest Medical Group's network are intended for non-urgent medical care. Emergency medical care should be directed to a local urgent care center or hospital or by dialing 9-1-1.

Prescription Medication Routing

Some patients may have the option to have their physician prescribed prescription medication, medical devices and/or nutritional supplements filled by their own pharmacy or through one of Southwest Medical Group's network pharmacies. Patient may easily request prescription reneweals and request transmission of your prescription to your preferred pharmacy with 24x7 online access.

Follow Up Care

Patients who subscribe to Southwest Medical Group's network will enjoy the freedom of not having to make annoying and time consuming phone calls and visits to the doctor's office for non-urgent medical care. In most cases follow-up patient care can be provided to the patient through private telephone and/or secure email physician consultations. No more long lines and germ filled waiting rooms. Patients may request a physician consultation 24/7 by accessing their account online.
